Pathways Recovery is now a 3rd party provider for most major health insurance companies.  If you have private insurance, tell the detox counselor when you call to discuss your treatment.  We will gather the information we need to check for benefits and let you insurance 106x150 Insurance Coverage For Detoxknow if your detox treatment can be reimbursed by your insurance carrier.

When you call to schedule your detox treatment, have your insurance card handy and be prepared to provide the counselor with the following information so that we may check for coverage:

  • Your Social Security Number
  • Insurance Company
  • Provider Phone Number
  • Member Name (father, mother, spouse)
  • Member Social Security Number
  • Member Date of Birth
  • Member ID Number
  • Group Name or Number

Ideally we would like you to e-mail or fax us a copy of the insurance card.  We need copies of the front and the back of the card.  The e-mail address to send the card to is info@pathwaysrecovery.com or you can fax it to (877)494-5088.  During normal business hours (8-5, Monday to Friday) our benefit check process will take only a couple of hours.
