Virtual Intensive Outpatient Program (IOP)

Virtual IOP at Pathways Recovery is a 9-hour-per-week telehealth program for adults with substance use disorders and co-occurring mental health conditions. Sessions run 3 hours, 3 days per week, for 6 weeks via HIPAA-compliant video. We accept Kaiser Permanente, Aetna, Cigna, Blue Cross Blue Shield, Anthem, and most major insurance.

Who Virtual IOP is Right For

Virtual IOP at Pathways Recovery is the right level for adults who need structured, clinically intensive addiction treatment but cannot or choose not to attend in-person sessions. It covers the same clinical content as our in-person IOP, delivered entirely through secure video.

The most common profiles are adults stepping down from residential or PHP who have returned home but are not yet ready for standard weekly outpatient, working professionals whose schedules do not accommodate in-person programming, adults living outside the Sacramento area who want access to Pathways’ clinical program, and adults managing co-occurring conditions such as anxiety, depression, bipolar disorder, or PTSD alongside substance use.

Virtual IOP is not the right level for anyone in active withdrawal or who requires daily clinical contact. Anyone in that situation should start with medical detox or residential treatment before stepping down to virtual IOP.

Virtual and in-person IOP deliver the same 9 hours of weekly clinical programming. The right choice depends on your location, schedule, and preference for remote vs. in-person care. Many people step from residential treatment directly into virtual IOP when they return home outside the Sacramento area.

What Virtual IOP Looks Like at Pathways Recovery

Virtual IOP at Pathways Recovery runs 3 sessions per week, 3 hours per session, for a total of 9 clinical hours weekly over 6 weeks. Sessions take place via HIPAA-compliant video and follow the same structure as our in-person program: each session opens with a group check-in, moves into CBT or DBT skill-building, includes a mindfulness or trauma-focused component, and closes with a peer accountability group.

Groups are capped at 8 participants. The small group size maintains the clinical contact and accountability that larger virtual programs cannot provide.

Morning and evening tracks are both available. The evening track runs Monday, Wednesday, and Friday and is built for working professionals. Both tracks deliver the same clinical content on the same 6-week timeline.

Individual therapy runs alongside group programming: one 50-minute individual session per week with your assigned licensed clinician. If you are using Suboxone as part of medication-assisted treatment, MAT management is integrated into the virtual program and does not require a separate in-person appointment.

Most people complete virtual IOP in 6 weeks. People with more complex co-occurring conditions or higher relapse risk may continue beyond 6 weeks at clinical discretion.

Therapies used in Virtual IOP

Virtual IOP at Pathways Recovery uses the same evidence-based therapies as our in-person program, adapted for telehealth delivery.

Cognitive Behavioral Therapy (CBT) is the clinical foundation of the program. CBT identifies the thought patterns and behavioral triggers driving substance use and builds concrete coping strategies to replace them. CBT translates directly to virtual formats and is the most studied modality in telehealth IOP research.

Dialectical Behavior Therapy (DBT) is integrated for people struggling with emotional regulation, impulsivity, or relapse driven by intense emotional states. DBT teaches distress tolerance, emotional regulation, mindfulness, and interpersonal effectiveness skills, practiced across group sessions each week.

EMDR (Eye Movement Desensitization and Reprocessing) is available for people with trauma histories. EMDR is clinically validated for telehealth delivery and is conducted by a trained clinician in individual sessions when indicated.

Brainspotting is offered for people with underlying trauma or emotional blocks that have not responded to talk therapy alone. Sessions are conducted one-on-one via secure video by a trained clinician.

Motivational Interviewing runs throughout the early weeks of the program to strengthen commitment to change and resolve ambivalence about continued treatment.

Mindfulness and Meditation groups are led by a licensed mental health professional in a structured group format. Mindfulness practice builds present-moment awareness and reduces the reactivity that drives cravings and relapse.

Substances We Treat in Virtual IOP

Virtual IOP at Pathways Recovery treats addiction to alcohol, opioids including fentanyl and heroin, benzodiazepines including Xanax and Klonopin, methamphetamine, cocaine, prescription stimulants including Adderall, and prescription painkillers including Opana and methadone. Co-occurring polysubstance use is treated alongside the primary substance use disorder.

Frequently Asked Questions About Virtual IOP

What is virtual IOP?

Virtual IOP is an intensive outpatient program delivered entirely through HIPAA-compliant video. It provides the same clinical care as in-person IOP, including CBT, DBT, EMDR, group therapy, individual sessions, and medication management, without requiring travel to a facility.

Is virtual IOP as effective as in-person IOP?

Yes. Multiple peer-reviewed studies show no significant difference in outcomes between telehealth and in-person IOP for substance use disorders, with comparable retention and symptom reduction rates. Quality of the clinical team and the right level of care matter more than format.

Does insurance cover virtual IOP?

Most commercial plans cover telehealth IOP under the same benefits as in-person IOP. We accept Kaiser Permanente, Aetna, Cigna, Blue Cross Blue Shield, Anthem, and most major carriers. Call (916) 735-8377 or submit the verification form to confirm your benefits in 15 minutes.

What do I need to participate?

A device with a camera and microphone, a stable internet connection, and a private space for sessions. Your clinician walks you through platform setup before your first session.

What is the schedule for virtual IOP?

Virtual IOP runs 3 sessions per week, 3 hours per session, for 9 clinical hours per week over 6 weeks. Morning and evening tracks are available, with the evening track running Monday, Wednesday, and Friday for working professionals.

Who is virtual IOP right for?

Can I do virtual IOP while working full-time?

Yes. The evening track runs Monday, Wednesday, and Friday evenings and is built specifically for working professionals. Most people continue working throughout virtual IOP.
